Passport Problems

If you have passport problems, what do you do?

  • If your passport is lost or stolen report it immediately. This is one of the most serious and potentially complicated passport problems you can have. If you are overseas, you will not be allowed to return to the U.S. until you are issued a new passport.
    • If you are overseas, contact the nearest U.S. Embassy or Consulate.
    • If you are in the U.S., you can report your passport lost or stolen by phone at 1-877-487-2778.
    • You can also report your passport as lost or stolen using Form DS-64:
    • Complete, sign and submit Form DS-64: Statement Regarding a Lost or Stolen Passport to:

      U.S. Department of State
      Passport Services
      Consular Lost/Stolen Passport Section
      1111 19th Street, NW, Suite 500
      Washington, DC 20036

    • To replace your lost or stolen passport, submit the following forms in person at a Passport Agency or Acceptance Facility:

      • Form DS-11: Application for a U.S. Passport, and
      • Form DS-64: Statement Regarding a Lost or Stolen Passport


      Passports reported lost or stolen by telephone or by submitting Form DS-64 are invalidated and can no longer be used for travel.

      If you recover the passport after you have reported it lost or stolen, please submit it to the address listed above.

      Once a passport is reported lost or stolen, it cannot be re-validated.

  • How do I change or correct information on my passport?

    • If you have a spelling or printing error on your passport:
      • You can have these errors corrected anytime without charge.
      • Submit completed Form DS-5504 to the address shown on the form.
    • If you have changed your name since your passport was issued:
      • If it is less than one year since your passport was issued you will not be charged.

      • Submit completed Form DS-5504 to the address shown on the form.

      • If it is more than one year since your passport was issued you will be charged a fee.

      • Submit completed Form DS-82 and the fee to the address shown on the form.

      • If you cannot legally document your name change, you will need to apply in person at an Acceptance Facility or a Passport Agency and you will be charged a fee.

      • You will need to complete DS-11 and bring additional documents verifying your identity and citizenship.


  • If you have less than 2-4 empty visa pages remaining in your passport, you can request additional pages.
    • Mail your current passport and
    • Completed Form DS-4085, Application for Additional Visa Pages to the address listed on the form.
    • There is a fee for this service.
